Indian Army Performs India’s First 3D Flex Aqueous Angiography Surgery

Landmark Ophthalmic Achievement by the Indian Army

In a significant breakthrough for eye care in India, the Indian Army has successfully conducted the country’s first 3D Flex Aqueous Angiography with iStent implantation . The pioneering glaucoma procedure was performed at the Army Hospital (Research & Referral), Delhi Cantt , placing the Armed Forces Medical Services at the forefront of advanced ophthalmic innovation.

Precision Surgery Using Cutting-Edge Technology

The surgery was led by Brigadier Dr Sanjay Mishra , Head of the Ophthalmology Department at the hospital. The procedure involved real-time, three-dimensional visualisation of the eye’s aqueous humour drainage system , which plays a crucial role in regulating intraocular pressure. Based on the live imaging, a tiny iStent micro-implant was accurately placed to improve fluid outflow and reduce pressure inside the eye—an essential step in preventing optic nerve damage caused by glaucoma.

Role of Advanced Imaging Systems

The success of the procedure was enabled by a movable stand-mounted Spectralis imaging system integrated with a 3D operating microscope . This setup offers surgeons high-definition, magnified, real-time visuals of the eye’s drainage channels during surgery. Such enhanced visual clarity allows highly targeted intervention, improving surgical precision and long-term outcomes for patients.

Significance for Glaucoma Management

Glaucoma is one of the leading causes of irreversible blindness in India , often progressing silently until advanced stages. The introduction of 3D Flex Aqueous Angiography allows early identification of dysfunctional drainage pathways and enables customised surgical planning. This advancement is expected to slow disease progression and significantly preserve vision.

Expansion of Advanced Care

Following the success at Delhi Cantt, the Armed Forces Medical Services plan to deploy the same technology at Command Hospital, Lucknow , which serves nearly 3.5 lakh patients across six states and Nepal, greatly expanding access to advanced glaucoma care.
