  1. Beneficiaries:
    The scheme covers all state government employees, retired government employees, and their dependent family members.

    Beneficiaries can avail of cashless medical services in both government and Ayushman Bharat-empanelled private hospitals across Uttar Pradesh.

    • In private empanelled hospitals, each beneficiary family is entitled to cashless treatment up to ?5 lakh per year.

    • In government medical institutions, cashless treatment is available without any financial limit.

  4. Empanelled Hospitals:
    The scheme is linked with hospitals empanelled under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), ensuring a wide network of quality healthcare providers.

  5. Mandatory State Health Card:
    To avail of the benefits, every eligible beneficiary must possess a State Health Card, which serves as proof of identity at empanelled hospitals. Cashless treatment is provided after verifying the beneficiary’s identity through this card.
